Health and Wellness/Evidence-Based Intervention Programs

These free Health and Wellness Programs offer prove techniques and tools to promote overall health and prevent disease among older adults and adults with disabilities. These workshops are evidence-based models that have been tested and proven to provide practical and effective strategies that empower adults to actively manage their health and take the steps necessary to live healthy fulfilling lives and reduce their health cost. Classes are highly participative, where mutual support and success build the participants’ confidence and empower them to improve their overall well-being. Participants will receive free materials and resources.

Diabetes Self-Management Program
This is a 6-week, once a week workshop that helps those living with diabetes or pre-diabetes to improve their general health by learning and finding practical ways to deal with symptoms of diabetes, including fatigue, pain, hyper/hypoglycemia, stress and difficult emotions.

Chronic Disease Self-Management Program
This is a 6-week, once a week workshop that help participants with chronic conditions learn to find practical ways to deal with pain, fatigue, stress, and how to get a good night's sleep. Learn ways to discover better nutritional and exercises choices.

Chronic Pain Self-Management Program
This is a 6-week, once a week workshop that helps participants, and their support person, to deal with the ongoing issues associated with chronic pain.

A Matter of Balance
This is a 8-week, once a week workshop proven to reduce the fear of falling and improves activity levels in older adults. It uses practical coping strategies to reduce fear of falling and to diminish the risk of falling.

Tai Chi for Arthritis and Fall Prevention
This workshop is recommended by the Arthritis Foundation of America and the Center for Disease Control and Prevention as effective and proven movements that will increase muscular strength support to protect joints. It will reduce pain, and improve balance both mentally and physically to reduce falls , improve health and wellness. Also available workshop offering is Seated Tai Chi for Arthritis.
